Your Family Portal allows you to securely access information related to your care services, stay connected with your care team, view important documents, manage billing, and access resources provided by your agency.
When your account is created, you will receive a Welcome Email from your agency containing instructions to access your Family Portal. This email includes your:
Username
Temporary Password
Email Address associated with your account
Step 1: Open your web browser and go to:
[Add Agency Portal URL here]
Example: agencyname.smartcaresoftware.com
Step 2: Enter your:
Email Address or Username (your username is your first and last name)
Example: michaeljackson
Password
Step 3: Select Log In.
If you do not remember your password:
Select Request Password Reset on the login screen.
Enter the email address associated with your account.
Check your email for password reset instructions.
Follow the prompts to create a new password.
Once logged in, you will have access to several sections within your portal. The information available may vary depending on the services provided by your agency.
The Quicklinks bar at the top of your page allows you to quickly view:
See and send messages
View your schedule & shift signatures
Rate a caregiver
Update your contact information
Update your profile picture
Change your password
Edit personal settings
The Memories section allows you to view photos and special moments shared by your care team.
In this section, you can:
View photos uploaded by caregivers
Ability to upload photos
Stay connected to your loved one's daily activities
The Caregiver Forms section contains forms and documentation completed by caregivers.
In this section, you can:
[add or delete what is viewable based on your agency]
Review completed tasks, scheduled activities, financial logs
Access completed forms related to care services
The Client Manager Forms section contains forms and documents completed by your Client Manager.
In this section, you can:
Review your client profile, priority change of care
Access scheduled tasks for your shifts
View admittance/discharge records
The Billing section allows you to manage invoices and payments.
In this section, you can:
View current and past invoices, and download invoice copies
View timesheets
Add payment methods & make secure online payments
View your deposit status (if your agency collected one)
The Feedback section allows you to share your experience with your agency.
In this section, you can:
Submit suggestions or concerns
Provide feedback about services received and rate your caregivers
Help your agency improve the care experience
The Resources section contains helpful information provided by your agency.
In this section, you may find:
